Sports Fund for Adapted Recreation

United Cerebral Palsy Association of Greater Indiana (UPCI Foundation) provides advocacy services to adults and children with cerebral palsy. The UPCI foundation administers a DL Sports Fund. The purpose of this fund is to: “provide financial assistance for the purchase of adapted recreation and sports’ equipment for children and adults with cerebral palsy, increase public awareness about the potential of children and adults with significant physical disabilities  to participate in sports and recreation activities, and promote adapted and inclusive sports for children with significant disabilites”.

This fund is for individuals in the State of Indiana with cerebral palsy. Individuals or families who are requesting these fund must have exhausted all other funding sources prior to approval. These approvals will be determined on a sliding scale based on the adjusted gross annual income of the household. The maximum approval amount per item or request is $800. Applicants must complete an application and include the required documentation. The required documentation would be a recommendation from a professional for the requested equipment or expense . The approved professional would be: a therapist, doctor, social worker, teacher, recreation therapist, physical educator, or an individual certified to teach/coach/supervise the activity. An applicant would need to provide a written estimate from the vendor that would include the vendors name, address and telephone number. Lastly, the applicant would need to provide proof of income which could be a pay stub, tax return or SSI authorization.

These applications are reviewed by the DL Sports Fund Steering Committee on the third Monday of February, May, August and November of each year and will be reviewed more frequently if necessary.
